Dancing is an activity in which, despite preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of injuries. As a participant in the program, I acknowledge that there are certain risks of personal injury and I agree to voluntarily assume those risks and responsibilities which I may sustain as a result of participating in any or all activities connected or associated with such a program. I understand that attending in-person classes, despite strict health & safety measures put in place, entails the potential risk of exposure to COVID-19 or other illnesses.
By attending in-person classes and events, I consent that I was not instructed by public health officials to self-isolate.
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the Studio and such exposure or infection may result in personal injury or illness. I understand the risk of becoming exposed may result from the actions, omissions, or negligence of myself and others. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself. I hereby release, covenant not to sue, discharge, and hold harmless the studio, its instructors, employees, and partners for any claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.
I agree that if I, at any time, test positive for COVID-19, I will notify MVNT Inc. immediately and will not participate in any in-person classes or events until I have finished the quarantine period and have permission from public health to resume normal public activities. I release all claims which may arise against, and agree not to sue MVNT Inc. and its officers, directors, agents, employees, and authorised volunteers from any and all claims by other parties resulting from physical or mental injuries, damages, and losses caused by me arising out of, or in any way associated with the activities of the program. I understand that I may be featured in photography and videography taken on the premises, collaborative events, and virtual platforms and I permit MVNT Inc. and its agents to use this footage for promotional purposes. I understand that all class packages are non-refundable, non-transferable and have an expiration date according to the package purchased, as stated on our website.

I understand that any changes resulting from special situations are subject to an administration fee. I give permission to MVNT Inc. to automatically charge my credit card on file for any late cancel/no-show infringements according to the outlined policy.
